Community Health Workers (CHW) are a vital link to promoting health and wellness throughout communities and to lowering healthcare costs. Credentialed CHWs act as part of a healthcare team within hospitals, public health and social service agencies and other community support organizations. They promote health and wellness through outreach, education, referral and follow-ups, case management, advocacy and home visiting services. The course has 66 contact hours.

Requirements

• Immunization records (if required for field work assignment)
• High school diploma or equivalency
• Prior experience in the field of healthcare or behavioral health-related services - Preferred
  

  Course Learning Outcomes

  Students will gain the knowledge, skills, and abilities as indicated by completing written and performance exams at the end of the course. Core competencies include, among others, motivational interviewing, professional skills, outreach, and advocacy. During the internship (where required) each student will work with a diverse population of people within a selected community by promoting public health, conducting wellness outreach services, educating individuals and families on health and wellness matters, and making needed referrals.
